Boys & Girls Club of Puerto Rico announces the\u00a0Child and Adult Care Food\u00a0Program (CACFP). In compliance with federal law and the civil rights policies and regulations of the United States Department of Agriculture (USDA), this institution is prohibited from discriminating on the basis of race, color, national origin, sex, age, disability or retaliation for past activities related to civil rights (not all prohibition principles apply to all programs). Food services are available to all enrolled children.<\/span><\/div>\n

Program information may be available in languages \u200b\u200bother than English. Individuals with disabilities who require alternative means of communication to obtain information about the program (for example, Braille, large print, audiotape, and American Sign Language) should contact the responsible state or local agency that administers the program or the USDA TARGET Center at (202) 720-2600 (voice and TTY) or contact USDA through the Federal Information Relay Service at (800) 877-8339.<\/span><\/p>\n

To file a discrimination complaint, the complainant must complete the USDA Program Discrimination Complaint Form (AD-3027), available online at\u00a0<\/span><\/p>\n

https:\/\/www.usda.gov\/sites\/default\/files\/documents\/usda-program-discrimination-complaint-form.pdf<\/a>, at any USDA office, by calling (866) 632-9992, or send a letter to the USDA. The letter must contain the name, address, and telephone number of the complainant, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) of the nature and extent of date of the alleged violation of civil rights. The letter or form AD-3027 should be submitted to USDA by:<\/span><\/p>\n
